What Is Inference-Based CBT for OCD?
Inference-Based CBT is a specialized, evidence-based approach to treating OCD that works a little differently than more familiar exposure-based models. Instead of starting with exposure to feared situations, I-CBT starts one step earlier — with the reasoning process that creates obsessional doubt in the first place.
People with OCD often experience what researchers call "inferential confusion": a tendency to trust an imagined possibility ("what if I left the stove on?") over what their senses and common sense are actually telling them ("I checked, it's off"). That gap between felt reality and imagined possibility is where obsessions take root. I-CBT helps clients recognize this reasoning pattern, understand how it develops, and rebuild trust in their own direct experience — which naturally reduces the doubt that compulsions are trying to resolve.
Because I-CBT targets the reasoning behind obsessions rather than relying primarily on exposure to distress, many clients find it to be a more approachable and collaborative starting point, particularly for those who have struggled with, or felt hesitant to begin, traditional exposure-based treatment.
At May You Find Peace, I-CBT is offered within our broader whole-person model of care, meaning it's never delivered in a vacuum. Depending on your needs, it may be complemented by:
- Mindfulness-based strategies to support present-moment awareness and self-trust
- IFS-informed work to gently understand the protective "parts" of you that OCD often speaks through
- EMDR, when trauma or distressing life experiences are intertwined with obsessive-compulsive patterns
